The New Mexico Shared Community Health Needs Assessment Collaborative (NMSCHNA) recognizes the value of working together to assess our communities and share findings to improve our health.

 

The NMSCHNA Collaborative is a dynamic public-private partnership that creates Shared Community Health Needs Assessment Reports, engages and activates communities, and supports data-driven health improvements for New Mexico residents.

Who We Are

New Mexico Department of Health and Presbyterian Community Health are the backbone of the New Mexico Community Health Improvement Plan and System Alignment for Sustainable Action (CHIPS & SALSA). We aim to align and support New Mexico institutions in completing health assessments and generating meaningful and aligned implementation plans. Launch and execute a combined assessment in 2025.

The Role of the Steering Committee is to Review, Adapt, and Agree on short-term project plans and when possible - commit resources to the implementation of our shared vision of small wins toward a larger and longer-term of goal of increased alignment, resource sharing, and action in Community Health Assessment & Planning.

Collaborative meetings are made up of any and all representatives of organizations, agencies, or associations that are interested in forming this “Collaborative” to achieve shared Community Health Assessment and Community Health Planning in the state of New Mexico. This group is invited to participate in this dynamic public/private partnership to iterate and improve over many cycles to:

1) add new partners who participate and benefit and

2) increase efficiency and reduce duplication of efforts
